Abstract: Hypertension is a leading global cause of mortality, yet antihypertensive drugs frequently have adverse side effects. Traditional medicinal fungi like Monascus species contain beneficial metabolites such as monascin (MS) and ankaflavin (AK). This double-blind, placebo-controlled clinical trial evaluated the effects of Ankascin 568, a fermented product of Monascus purpureus NTU 568, in patients with mild to moderate hypertension. Participants received either a placebo or two 500 mg capsules of Ankascin 568 daily for 8 weeks. Results showed significant reductions in systolic blood pressure (from 141.6 ± 12.0 to 133.9 ± 14.4 mmHg, P < 0.05) and diastolic blood pressure (from 91.7 ± 8.1 to 84.8 ± 7.4 mmHg, P < 0.05). Ankascin 568 also significantly decreased serum triglycerides and total cholesterol, increased HDL-C levels, and reduced LDL-C levels, enhancing the lipid profile without adverse effects on liver or kidney functions. In conclusion, 8-week administration of Ankascin 568 significantly improved blood pressure and lipid profile, supporting cardiovascular health.
